If a replacement card is needed the fee is $10.00. For Essential Checking Customers: $10.00 per card ordered; $5.00 per card reissued. Please read and accept the terms and conditions below to get started.


These Terms and Conditions for My Custom Card (“Terms”) are separate from your other account documents and should be added to your files for future reference. By submitting one or more images to be considered for use on your debit card, each cardholder (“you”) agrees to the following Terms. Home Savings Bank reserves the right to add to or modify these Terms and to terminate the My Custom Card debit card program or any My Custom Card debit card issued to you at any time in its sole discretion. There is a $10.00 fee if a replacement card is needed. For Essential Checking Customers: $10.00 per card ordered; $5.00 per card reissued.

Image Requirements and Restrictions

You may not use images that contain any of the following:

  • Copyrighted material or other photographs not owned by you, including professional photographs taken by others.
  • Product brands and trademarked material.
  • Obscene or lewd material, harassing or discriminatory material, provocative material, or any other socially unacceptable material.
  • Currency or flags of any country or flags or symbols of any organization.
  • Phone numbers, web addresses, e-mail addresses, or other personal information.
  • Symbols of death, destruction, violence, or any other negative images that do not reflect positively on Home Savings Bank.
  • Celebrities and public figures, including sports, music, entertainment, and political figures.
  • Political statements.
  • Reference to MasterCard®, American Express®, Discover® or any other payment brand.
  • Profanity, obscenities or nudity.
  • Alcohol or tobacco.
  • Religious symbols, images and text.
  • Images of money.
  • Any content that might infringe, denigrate or dilute the VISA brand or its member financial institutions.
  • Photos that feature a mascot or character as the primary focus of the photo.
  • Images that may interfere with the acceptance of the Card or the security features. Images that do not cover the entire surface of the Card.

Home Savings Bank reserves the right to reject any image submitted in our sole and absolute discretion. You agree to indemnify, defend, and hold harmless Home Savings Bank and its board of directors, officers, employees, and contractors from and against any damages, costs, expenses, including court costs and attorney’s fees, arising out of or related to your acts or omissions or any claim, action, or dispute, whether well-grounded or baseless, that an image submitted by you violates the rights of any person or entity or applicable law.

A custom image on your card does not imply any additional security and when appropriate you should expect to show photo ID if requested. If you discover your debit card is missing/stolen and are able to call during regular business hours (M-F 9am- 5pm), please call one of our friendly bankers at 608.282.6000 or 800.282.5115 . They will assist you in filling out a Lost/Stolen Report and getting you a new card. If you discover your debit card is missing/stolen outside normal business hours, please call 1.800.554.8969 and follow the prompts. Visa will then place a hold on your card. The following business day please call one of our bankers at 608.282.6000 or 800.282.5115 for help ordering a new debit card. If you believe your debit card has been stolen please remember to contact the police.

Reissued Cards

Debit cards are reissued under normal use every 3 years. Once your debit card has been issued with a custom photo, subsequent reissues of the card whether reissued due to the card being lost or stolen or if the card expires, will be issued with the image previously uploaded. Should you wish to upload a new photo to be used on your card reissue, please do so at least two months (60 days) prior to your card expiration date. Do not upload a new photo if fewer than two months remain until your card expires. You agree to destroy your old card once you receive your new Home Savings Bank My Custom Card debit card and hold Home Savings Bank harmless from any loss as a result of not doing so.

Cancelling a My Custom Card Debit Card

If you do not wish to have your debit card reissued with a custom image, please contact us at 608.282.6000 or toll free 800.282.5115 at least two months (60 days) prior to your card expiration date.


These rules govern the relationship between Home Savings Bank and each person (Customer) who is issued/ authorized to use a Home Savings Bank Debit card.


The following definitions apply to these Rules:

“ACCOUNT” means your checking or savings account with Home Savings Bank.
“CARD” means your Home Savings Bank Debit Card issued by Home Savings Bank and any additional cards or renewals.
“WE”, “US” and “OUR” means Home Savings Bank.
“YOU”, and “YOUR” or “CUSTOMER” means each person who is a party to the Account and who applied for the card.
“ATM TERMINALS ” or “AUTOMATIC TELLER MACHINE” means any terminal at which you may use your Card together with your Personal Identification Number (PIN) to perform transactions.

Terminals and the Cards

Subject to restrictions as may be imposed by the Bank, the Card may be used by Customer at any Terminal the Bank may designate to perform any of the transactions listed on the disclosure which the Terminal will perform and in connection with each checking, savings and/or credit card account of Customer with the Bank or the Card issuer which Customer has requested to use with the Card and the Bank has approved (collectively the “Account”).

Withdrawals, Transfers, Cash Advances

Each Transaction requested by Customer at a Terminal:

a. Which consists of a withdrawal or transfer from an Account, other than by Authorized Check, is an order to the Bank to pay from that Account at that time, which the Bank may charge against that Account, even though the charge creates an overdraft. If the Account is maintained in connection with overdraft checking protection, the charge may, in accordance with the agreement governing that account, result in an extension of credit to Customer.

b. Which consist of a cash advance from credit card is an order to the Bank or the Card issuer to make such cash advance.


All checks or drafts which the Bank has authorized through a Terminal for deposit of payment are credited subject to collection and the credit will be reversed if the check or draft is not paid. Except for cash deposits at a clerk-operated terminal for which immediate credit will be given, all cash deposits and payments are received subject to verification by the Bank and need not credit the Account for deposits until the bank actually receives the funds. If the reversal of a deposit overdraws the Account, all or part of the deficiency may be charged to any other account at the Bank of the Customer authorized to use the Card including any necessary extension of credit and Customer shall pay to the Bank any deficiency which cannot be paid out of such accounts.

Limits on Debit Card Transactions

You may use the Card only if the available balance in your Account (together with your unused credit line if your Account has an overdraft credit line associate with it) will be sufficient to pay the amount of the Purchases, Cash Withdrawals and ATM transactions. Any Purchase or Cash Withdrawal may be subject to authorization by us or by a MasterCard or VISA authorization center and no authorization will be given if the amount of all outstanding Purchases and Cash Withdrawals will exceed your available balance or your applicable credit limit. At Point of Sale terminals, you may make only ten (10) transactions each date to pay for purchases totaling up to $425.00 per day for pinned transactions, or $2,500 per day for signature-based transactions.

Authorization to Debit or Credit Accounts

Each time your Card is properly used you authorize Home Savings Bank to debit or credit your Account (whichever is appropriate) for the total amount shown on any sales draft, withdrawal order or credit voucher originated by use of the Card, whether or not signed by you, and Home Savings Bank is permitted to handle such sales drafts, orders and vouchers in the same way it handles authorized checks drawn on your Account.


If use of your Card overdraws your Account, you will be notified and you agree to make immediate payment to Home Savings Bank in the amount of any such overdrafts together with such service charges as Home Savings Bank may impose. Also, whenever your Account is overdrawn, we have the right to return unpaid any checks or other orders on your Account which are presented to Home Savings Bank and to assess a service charge for making such returns. If your Account has an overdraft line of credit associated with it the terms and conditions contained in your credit line Agreement (or other applicable agreement) will control with respect to overdrafts of your Account regardless of whether they result from Purchases, Cash Withdrawals, use of checks, or use of any other card or device.

Fees and Charges

There are no fees for the use of a debit card imposed by Home Savings Bank if used as a signature based credit transaction. (International transaction fees may apply) However, a fee may be charged for each withdrawal at an ATM or point of sale terminal. Please consult the Service Charge Fee Schedule. Fees may be reduced or waived for specific account relationships. When you use an ATM not owned by Home Savings Bank, you may be charged a fee by the ATM operator or any networks used to process the transaction, including a fee for a balance inquiry even if you do not complete a funds transfer.

Periodic Statements

Your regular monthly Account statement will reflect each Purchase and Cash Withdrawal charged to your Account and each credit applied to your Account during the monthly account cycle and the related fee, if any.

Transaction Receipts – You will get a receipt at the time you make any transfers to or from your Account using an ATM terminal, or make purchases using your Debit card as permitted by regulation. You may not receive a receipt if your transaction is less than $15.

Customer Liability- Notification of Unauthorized Uses

To best protect you as a customer, you should not authorize others to complete transactions for you, and if you do, please notify Home Savings Bank when future transactions are unauthorized. Immediately notify Home Savings Bank of any loss, theft, disappearance or known suspected unauthorized use of the Card. Such notice shall be provided by calling us at 608.282.6000 or writing us at Home Savings Bank, 3762 E. Washington Ave., Madison, WI 53704. Upon verbal request we shall ask you to confirm your verbal notice in writing. You shall be obligated to pay Home Savings Bank the amount of any money, property or services obtained by the authorized use of the Card to the extent that Home Savings Bank is unable to charge such amounts to the Accounts, and you hereby authorize the Bank to charge the amount of any such obligation to any other of your Accounts at Home Savings Bank.

Refunds on Purchases

Cash refunds will not be made to you on Purchases made with your Card. If a merchant who honors your Card gives you a credit for merchandise returns or adjustment, the merchant will do so by crediting your account. Unless your use of the Card results in an extension of credit, any claim or defense with respect to the property or services purchased with your Card must be handled by you directly with the merchant or other business establishment which accepts the Card and any such claim or defense which you assert will not release you of your obligation to pay the total amount of the sales draft plus any appropriate charges we may be authorized to make.

Foreign Transactions

If your Card is used to effect a transaction in a foreign currency, the transaction amount will be converted to U.S. dollars by VISA International and MasterCard International. VISA and MasterCard convert foreign currency to U.S. dollars using either the government-mandated exchange rate or the wholesale exchange rate in effect one day before the date of conversion, as applicable. The exchange rate is increased by one percent (1%) if the conversion is made in connection with a credit to an account. The date of conversion by VISA or MasterCard may differ from the purchase date and the posting date identified in the monthly statement for your Account. You agree to pay charges and accept credits for the converted transaction amounts in accordance with the terms of this section.

Limitations in Home Savings Bank’s Responsibility

Systems containing terminals, at which the card may be used, are available for the convenience of a customer and, except to the extent provided in Section 910 of the Electronic Funds Transfer Act, Home Savings Bank is not liable for the unavailability or failure to operate all or any part of any system. Except for its own negligence, Home Savings Bank is not liable for any personal injury or tangible property damage suffered or incurred by customer through use or attempted use by customer of the Card at any terminal. Except to the extent provided in Section 910 of the Electronic Funds Transfer Act, Home Savings Bank is not liable for any loss, cost, damage or expense incurred by customer by reason of malfunction of any part of any system or failure to complete any transaction which is caused by natural disaster, fire, strike, war, riot or act of God or any other cause beyond the control of Home Savings Bank and any other entity which is part of or connected to any System, and Home Savings Bank is not liable for consequential damages.

In Case of Errors or Questions About Your Electronic Transfers (Consumer Accounts Only):

Telephone us at 608.282.6000 or write to us at Home Savings Bank, 3762 E. Washington Ave., Madison, WI 53704, as soon as you can, if you think your statement or receipt is wrong or if you need more information about a transfer listed on the statement or receipt. YOU MUST CONTACT US ONLY AT THIS TELEPHONE NUMBER OR ADDRESS ABOUT YOUR ERRORS OR QUESTIONS. We must hear from you no later than 60 days after we sent you the FIRST statement on which the problem or error appeared.

  1. Tell us your name and account number.

  2. Describe the error or the transfer you are unsure about, and explain as clearly as you can why you believe it is an error or why you need more information.

  3. Tell us the dollar amount of the suspected error.

  4. It will be helpful to us if you also give us a telephone number at which you can be reached in case we need further information.

If you notify us orally, we may require that you send us your complaint or question in writing within ten (10) business days. Business days are defined as every day except Saturdays, Sundays and federal holidays.

We will tell you the results of our investigation within ten (10) business days after we hear from you and will correct any error promptly. If we need more time, however, we may take up to forty-five (45) days to investigate your complaint or question. If we decide to do this, we will re-credit your account within 10 business days, (five (5) for debit card transactions) for the amount you think is in error, so that you will have the use of the money during the time it takes us to complete our investigation. If, however you have told us orally about your question or complaint and we did not receive your written confirmation within ten (10) business days of your oral report (after we asked you for it), we will not provisionally re-credit your Account.

If we decide that there was no error, we will send you a written explanation within three (3) business days after we finish our investigation. You may ask for copies of the documents that we used in our investigation.

For new accounts we will have 20 business days to resolve an alleged error and provisionally credit the account and 90 calendar days to complete the investigation, rather than forty-five (45).

For point-of-sale transfers and foreign initiated electronic fund transfers we may take ten (10) business days, after we hear from you, to tell you the results of our investigation, or alternatively, re-credit your Account and resolve the error within ninety (90) days.

Unauthorized Transfers/ Notification of Unauthorized Use

Notify Home Savings Bank at once if a card and/or code has been lost or stolen at 608.282.6000 followed by a written notice mailed to Home Savings Bank, 3762 E. Washington Ave., Madison, WI 53704. If notice of a lost or stolen card and/or code is given, liability is the lesser of:

  1. $50.00, (Zero for certain debit card transactions)

  2. The amount of any money, property, or services obtained before notice was given of unauthorized use.

Also, if a statement shows unauthorized transfers, notify Home Savings Bank at 608.282.6000 immediately. Notice must be given within sixty (60) days after the statement was mailed. Any lost money will not be refunded after sixty (60) days if Home Savings Bank can prove unauthorized use could have been stopped if notification had been given. The time period will be extended with good reason such as hospital stays or long absence.

Limitations on Use of Card

Customer May Not:

  1. Request any transaction at a Terminal if Customer knows or is informed by or at the Terminal, or by a clerk, that the Terminal is malfunctioning or not operating.
  2. Attempt any transaction, without the use of a Card and insertion of Customer’s correct PIN which will be furnished by Home Savings Bank.
  3. Attempt to initiate any transaction at a terminal which requires the assistance of a clerk to complete, except on the same date and at the same place where Customer initiated the transaction.
  4. Attempt to initiate any transaction in connection with an account which has been closed by Customer or Home Savings Bank, or which is subject to legal process or other encumbrance.
  5. Request a withdrawal or transfer of funds from an account (i) if the withdrawal or transfer would overdraw the Account or, if the account is maintained in connection with a check credit or overdraft checking line and would exceed the credit limit of that line: (ii) a withdrawal or transfer in an amount in excess of any dollar limitations imposed by the terminal operator; or (iii) in an amount in excess of any limitations imposed by the terminal operator.
  6. Deposit funds to an Account (i) by means of anything other than transfer from another account, cash or a completed negotiable instrument payable in U.S. dollars dated within six (6) months prior to the date of the attempted deposit, drawn by or properly endorsed by customer and the amount of which does not violate a restriction on the instrument; or (ii) in an amount of excess of any limitations imposed by the terminal operator.
  7. Obtain authorization of a check or draft which is (i) drawn by or payable to the order of someone other than the customer presenting the card; (ii) not endorsed by the customer presenting the card (unless payable to a merchant); (iii) not payable in U.S. Dollars; (iv) dated more than six (6) months prior to the date of the transaction; (v)in violation of any state restriction on the check or draft (for example, not valid after thirty (30) days; or (vi) for an amount which exceeds the limits established and disclosed by the bank

Neither the Bank nor any Terminal owner is liable to Customer if the transaction is not completed and Customer has violated any of these rules.

Multiple Party Accounts

Each customer who is a party to an Account which is a joint Account hereby appoints each other Customer who is a party to such Account as customer’s attorney with power to appoint one or more agents with power to use the card to make withdrawals from such account. Each Customer shall indemnity the Bank for any tax it may be required to pay under Wisconsin statues by reason of withdrawals or payments from the account after the death of a customer, to any person or to the survivor in the case of a joint account.

Card Ownership; Termination

The Card remains the property of Home Savings Bank and shall be surrendered by Customer upon request; and the Bank may terminate Customer’s privilege of using the Card and may withhold approval of any transaction at any time.


These rules may be amended by the Bank any time and (subject to compliance with any applicable requirement of law concerning notice) such amendment shall be effective twenty-one (21) days after mailing by the Bank a copy of such amendment to Customer at the address to which Customer’s Statement are sent. If, however an immediate change in the terms and conditions is necessary for security reasons, we may amend these terms and conditions without prior notice.


Retention and use of Card constitutes acceptance of these Rules as amended from time to time. All terms, conditions and agreements which govern your Account (whether set forth in your Account Rules, in any related rules and regulations or otherwise) also apply to the Card except where these Rules provide differently. This agreement and your use of the card will be governed by the laws of the State of Wisconsin. These rules constitute an addition to all other agreements and regulations of Home Savings Bank governing Accounts which are accessible by the Card of indebtedness on which payments may be made by the use of the Card.
